Valid photo ID (driver's license or passport)
Insurance card(s) — health, auto, or workers' comp
Completed new patient intake forms (if emailed in advance)
List of current medications and supplements
Any previous imaging (X-rays, MRI, CT scans)

Workers' comp claim number (if applicable)

We accept most major health insurance plans and are in-network with many providers. We also accept auto accident insurance (PIP and liability) and workers' compensation — often at no out-of-pocket cost to you.
