Based on your evaluation, Dr. Cantu will explain his findings and present a personalized care plan — with realistic goals and a clear timeline.

Valid photo ID (driver's license or passport)

Insurance card(s) — health, auto, or workers' comp

Completed new patient intake forms (if emailed in advance)

List of current medications and supplements

Any previous imaging (X-rays, MRI, CT scans)

Police report (if auto accident related)

Workers' comp claim number (if applicable)

Referral from your primary care doctor (if required by insurance)

We accept most major health insurance plans and are in-network with many providers. We also accept auto accident insurance (PIP and liability) and workers' compensation — often at no out-of-pocket cost to you.

If you do not have insurance, we offer affordable self-pay rates and flexible payment options. We believe financial concerns should never be a barrier to quality chiropractic care.
